Lesson plans

How do you write out your lesson plans? I have 4 children (ages 5 to 12). Some things we do together such as history, bible time and science. Other subjects are grade specific such as math and spelling. Do you write a separate lesson plan for each child or is it general enough to cover everyone? I feel like I need to be specific for each child in case my husband has to take over for the day.
I've also heard of some moms keeping track of the books their kids read. Is that every book they read or just the ones for "school"?
Any suggestions would be greatly appreciated.
